How In-Home Wound Care Works

One call gets things started. We check the patient’s coverage, confirm availability in their area, and a licensed clinician who specializes in wound care comes to them.

Four steps

  1. Request a call

    Call us or fill out the short form. We ask for your contact details, where the patient is staying, and their insurance — no wound descriptions or medical history. We respond within 24 hours.

  2. We check coverage

    We verify the patient’s coverage and confirm a clinician is available in their area. We accept Medicare and Medi-Cal, including patients who have both.

  3. A clinician visits

    A licensed clinician who specializes in wound care visits the patient wherever they are living — at home, in assisted living, or in a board-and-care home — and coordinates with the existing care team.

  4. Ongoing support and notes

    Clear clinical documentation after each visit. We work alongside existing nurses and home health providers.

What to have ready for the call

  • The patient’s insurance card or cards
  • The address where the patient is staying right now
  • The name of the patient’s primary doctor and any home health agency involved

Family members, caregivers, facility staff, and case managers can all request a call on the patient’s behalf.

One thing to know: coverage under Medicare Advantage and Medi-Cal plans varies by plan, so we’ll verify the patient’s specific plan before anything is scheduled. There’s no cost to find out. Go ahead and submit — we’ll confirm when we call.
