Wound Care Visits for Your Residents and Patients

For assisted living communities, board-and-care homes, home health agencies, case managers, and discharge planners. A licensed clinician who specializes in wound care visits the resident on site and shares notes after every visit.

Who we work with

  • Assisted living communities
  • Board-and-care homes (RCFEs)
  • Home health agencies and nursing teams
  • Case managers, social workers, and discharge planners

How it works for your team

Call us or submit the form below and choose “Facility staff” or “Case manager / Social worker.” We’ll call back within 24 hours to go over the resident’s location and insurance, and we verify coverage before anything is scheduled. If more than one resident needs wound care, mention it when we call.

A clinician visits the resident where they live. Clinical notes go to the resident’s existing providers and home health team after every visit, and we work alongside your staff and any nursing already in place.

Coverage

We accept Medicare and Medi-Cal, including residents who have both. For patients in a skilled nursing facility, wound care is usually covered by the facility under Medicare Part A, so we often can’t bill separately.

Notes Shared

Clear clinical notes go to the patient's existing providers and home health team, so everyone stays coordinated.
