Caring for Someone With a Wound That Won’t Heal

If you’re looking after a parent, spouse, or other family member with a wound, you can request a call for them. A licensed clinician who specializes in wound care can visit them where they live.

When it may be time to ask for help

  • The wound hasn’t clearly improved over several weeks
  • Dressing changes are hard to keep up with at home
  • Getting your loved one to appointments is difficult or exhausting
  • They were recently discharged from a hospital or skilled nursing facility with a wound that still needs care
  • You feel stuck and want a next step or a second opinion

What the visit looks like

A clinician comes to your loved one — at home, in assisted living, or in a board-and-care home — and coordinates with their existing doctors and home health team. Clinical notes are shared after every visit, so everyone stays on the same page.

This service is not for emergencies. If a wound is spreading quickly, the skin around it turns dark, or the patient has a fever, new confusion, or seems very unwell, call 911 or go to the nearest emergency room.

A Clinician Visits

In most cases, a licensed clinician who specializes in wound care visits the patient in person — at home, in assisted living, or wherever they are.

One thing to know: coverage under Medicare Advantage and Medi-Cal plans varies by plan, so we’ll verify the patient’s specific plan before anything is scheduled. There’s no cost to find out. Go ahead and submit — we’ll confirm when we call.
A quick note: for patients in a skilled nursing facility, wound care is usually covered by the facility under Medicare Part A, so we often can’t bill separately. You’re still welcome to submit this form or call us — we’re happy to talk it through and point you in the right direction. (949) 424-5126

Common Questions

Yes. Family members, caregivers, facility staff, and case managers can all request a call on the patient’s behalf.
The patient needs Medicare, Medi-Cal, or both. If the patient has a Medicare Advantage plan or a Medi-Cal managed care plan, tell us the plan name — coverage under those plans varies, so we’ll verify it before anything is scheduled. Not sure what the patient has? Call us and we’ll help you check.
That is a good sign. We work alongside existing home health agencies and nursing teams. Our providers coordinate care and share clinical notes with the existing team.
We contact you within 24 hours of your request. In most cases a clinician visits the patient within 72 to 96 hours, depending on location and availability.
Your name and phone number, the patient’s ZIP code, your relationship to the patient, where the patient is staying, and their insurance. We don’t ask for wound descriptions or medical history on the form — we go over those on the call.
